Dear Barry,

I wonder if you have read the article in the November 4, 2003 issue of the ASHA Leader put out by the American Speech-Language-Hearing Association Vol. 8 No. 20. Their website is www.asha.org. The article is entitled Clinical Management of Tinnitus and Hyperacusis. Of interest is their treatment recommendation. They work above the source of tinnitus: therefore, the etiology of the tinnitus is irrelevant. They suggest Habituation to the tinnitus signal which is most effectively achieved by the combination of two components: 1) directive counseling, which attempts to neutralize the negative emotional associations with the tinnitus and 2) sound therapy, which interferes with the detection of the tinnitus signal. Hi Linda;

The therapy ASHA is referring to is Tinnitus Retraining Therapy (TRT) which has been around for some time. It is very helpful for many people although it does take time and commitment. There are numerous websites on TRT which can be found on Google at http://www.google.com/search?hl=en&ie=UTF-8&oe=UTF-8&q=tinnitus+retraining+therapy.

Another medical treatment that works above the source is Dr. Shulman’s use of Neurontin and Klonopin to treat the brain receptors responsible for tinnitus. You may have seen our article in a previous newsletter.

I think there are many reasons to be hopeful. This month's newsletter contains an article on Sound Cancellation Therapy for Tinnitus that is being developed at Columbia University. This consists of pitch and loudness matching of an individual’s tinnitus. The exact tinnitus sound is then fed into a computer that generates the same sound but shifts the phase so it cancels the tinnitus sound. This sound is then burned into a CD for the patient’s use.

Arches Tinnitus Formulas work very well in conjunction with all the above therapies. These therapies and ATF will potentiate each other so the result will be better than by using one of them alone.

Hi Barry

I have not had a chance to talk to anyone else who suffers from tinnitus. I am just curious to know if it is normal for it to change from day to day. I have it it for 13 months now. Sometimes it is very quiet and I hardly notice it and other days it is so loud I can't stand it. Sometimes at night it almost disappears and other nights it keeps me awake! Every morning I wake up wondering what kind of "Tinnitus Day" am I going to have! Sometimes one ear and sometimes both!

I visited a friend a couple of weeks ago, who had a very very yappy small dog and it barked continuously for about 20 minutes before she took it to another room. I should have said something, but now I worry it damaged my ear even more, as I have not had a really good day since then. Is it possible that something like that can cause even more damage? Do you think my ear will settle down eventually?

Dear Susan;

The majority of people with tinnitus, myself included, have a fairly constant sound level. Many people, however, report that their tinnitus fluctuates dramatically as yours does. What is critical is to find the trigger(s) that makes your tinnitus increase in intensity. The trigger can be different for everyone but stress is the number one aggravator of tinnitus. It’s not uncommon for stress to turn a level three tinnitus (on a scale of 10) into a level seven. Another major aggravator is salt. And for some people its sugar.

Try to monitor your emotions, stress level, diet, work schedule, etc to see if you can correlate the increase with something else. If you can find the trigger, it will be very helpful.

I don’t think the small dog did any serious damage to your ear. It should calm down. You’re probably terribly upset about the incident and the stress resulting from it could be the factor that has kept your tinnitus high. It’s so important to understand and moderate stress levels, I can’t overstate it.

My twin brother and I have suffered light to medium Tinnitus for approximately four years. My twin, because of personal stress has had more severe bouts.

He recently tried Odourless Garlic capsules and found, so he said, measurable relief. Do you feel that there is any truth in the use of Garlic. He also believed taking these Odourless Garlic capsules helped to clear his sinuses and Eustachian tubes.

I am a big fan of garlic and think it’s one of the best things you can do for yourself. We have odorless garlic in our Arches Tinnitus Relief Formula. Garlic is helpful in reducing stress and cholesterol and increasing circulation. All of these are helpful in reducing tinnitus symptoms.

Your comment about garlic cleaning his sinuses and Eustachian tube may be the key here. It’s well known that sinus problems can aggravate, if not cause, tinnitus.

The best form of garlic is pure, not deodorized. The parts of garlic that cause the odor are sulfur compounds. This is what’s removed during the deodorizing process. Unfortunately, these compounds are also the ones with the most beneficial effects. The manufacturers of odorless garlic say they have created a precursor to these compounds that doesn’t have the odor. This may be true but I believe whole garlic cloves are the best choice. Just make sure everyone around you also eats them.
